Documents that are required to be completed for registration can be found in the New Parents drop down menu.

Please remember, you must have ALL paperwork completed and turned in with the registration fee to be enrolled in the program.

Please be sure to read the instructions on the forms completely!

Parents must complete/provide the following documents

Child Information Card (for each child)
Driver’s license
Signed food policy
All purpose permission/acknowledgement form
Written Information packet Documentation
Parent Notification of Licensing Notebook
Annual pest management notification
Family Registration Form
Billing Contract (completed with the director)

Parents must complete for children below school age

Immunization record
Health appraisal (within 30 days of initial attendance)

Parents must complete the following for School aged children

Statement of Immunization and Good Health

We have full and part time spaces available!

We are enrolling children ages 30 months to 12 years old.
The Center hours are 6 am - 6 pm Monday through Friday.
The center is open for tours and enrollment by appointment.

Immunization records are required to be on file the first day of attendance.
